Validez diagnóstica de la ecografía abdominal comparada con la colangiopancreatografía por resonancia magnética (CPRM) en la obstrucción de la vía biliar

Diagnostic validity of abdominal ultrasound compared with magnetic resonance cholangiography in biliary tract obstruction

Resumen

La obstrucción de la vía biliar es una condición frecuente asociada a complicaciones graves. La ecografía abdominal es el método inicial, aunque presenta limitaciones diagnósticas. La colangiopancreatografía por resonancia magnética (CPRM) ofrece mayor precisión.  Determinar la validez diagnóstica de la ecografía abdominal comparada con la CPRM en pacientes con sospecha de obstrucción de la vía biliar. Estudio prospectivo de precisión diagnóstica en 190 pacientes adultos con sospecha clínica y bioquímica de obstrucción biliar, incluidos de forma consecutiva. Se comparó la ecografía abdominal con la CPRM mediante tablas de contingencia 2×2. Se calcularon sensibilidad, especificidad, valores predictivos, razones de verosimilitud y curva ROC con intervalos de confianza del 95%.  La ecografía abdominal presentó una sensibilidad de 20,63% (IC 95%: 11,5–33,1) y una especificidad de 96,06% (IC 95%: 91,0–98,7). El valor predictivo positivo fue de 72,22% y el negativo de 70,93%. La exactitud diagnóstica fue de 71,05%, con un área bajo la curva ROC de 0,584.  La ecografía abdominal presenta alta especificidad, pero baja sensibilidad, lo que limita su uso como método diagnóstico único. La CPRM demuestra un rendimiento diagnóstico superior y debe considerarse el método de elección para confirmar la obstrucción biliar.Palabras clave: de tres a cinco palabras clave separadas por comas con primera letra en minúscula.
