Factors that limit compliance with advance directives in cancer patients in palliative care: Systematic review

La planificación y la toma de las voluntades anticipadas, garantiza una atención humanizada, con enfoque en sus creencias espirituales y culturales, con base en el respeto, en la autonomía, para una muerte digna. El objetivo de esta revisión fue analizar y determinar los factores que obstaculizan el cumplimiento de las voluntades anticipadas en pacientes oncológicos en cuidados paliativos. La búsqueda se realizó en: PubMed, Scielo y Google Scholar, se tomaron en cuenta criterios de inclusión y exclusión, para guiar el estudio se utilizó el método PRISMA 2020. Los resultados indican entre los principales factores que limitan el cumplimiento de las voluntades anticipadas se hallan: falta de comunicación entre las partes implicadas, desconocimiento sobre las voluntades anticipadas y su implementación, condiciones socioeconómicas y emocionales de los pacientes y familiares, no existe una categorización detallada de dichos factores, por tanto, en base a la información obtenida se plantea una propuesta de clasificación en este trabajo. Finalmente, las medidas aplicadas para reducir los efectos de los factores limitantes incluyen: estrategias comunicacionales entre las partes involucradas, educación y concientización pública. Este estudio concluye que existen diversos factores que afectan la planificación de voluntades anticipadas, basados en comunicación no efectiva entre pacientes, familia, personal médico, barreras socio culturales y emocionales, sumado a esto el bajo conocimiento sobre voluntades anticipadas. No existe una categorización clara de los factores limitantes, por lo cual es necesario estructurar una clasificación en futuras investigaciones. Además, es importante abordar estos desafíos, mediante la consecución de estrategias que susciten una mejor comunicación, educación y conciencia pública sobre las voluntades anticipadas.
